چکیده

TOPIC: Obstructive Lung Diseases TYPE: Original Investigations PURPOSE: Many patients with severe asthma driven by type 2 inflammation require long-term treatment systemic corticosteroids. Dupilumab, a fully human monoclonal antibody, blocks the shared receptor component for IL-4/13, key and central drivers of inflammation. Dupilumab is approved in US as an add-on moderate-to-severe aged ≥12 years eosinophilic phenotype or oral corticosteroid (OCS)–dependent asthma. In phase 3 OCS-sparing VENTURE (NCT02528214) study, dupilumab 300 mg every weeks (q2w) vs placebo significantly reduced OCS use adults/adolescents OCS-dependent protocolized fashion over 24 weeks. LIBERTY ASTHMA TRAVERSE (NCT02134028), single-arm, open-label extension evaluated dupilumab’s safety, tolerability, efficacy rolled from previous study. This post hoc analysis assessed ability to sustain reduction that was achieved during VENTURE, long term population OCS-dependent, METHODS: treated q2w who enrolled received 300mg up 96 Endpoints dupilumab/dupilumab-treated placebo/dupilumab-treated included mean dose throughout TRAVERSE. Additional endpoints proportion discontinued OCS, <5 ≥50% end maintained this duration Efficacy were exacerbation rate change pre-BD FEV1 baseline. RESULTS: Of 210 patients, 187 after completing VENTURE. At 27% compared placebo. By Week 48 TRAVERSE, reductions baseline observed 78% 53% these sustained (89%, dupilumab/dupilumab; 74%, placebo/dupilumab). 0 (48/90) 30% (29/97) use, 74% (67/90) 38% (37/97) mg, 82% (74/90) 54% (52/97) clinically meaningful use. Most discontinuation at Weeks Despite requirements, rates low improvements sustained. Risk of corticosteroid-related adverse events in asthma patients with high oral corticosteroid use.

BACKGROUND Oral corticosteroids (OCS) are a mainstay of asthma treatment. Their use increases the risk of various corticosteroid-related adverse events, but the extent of risk is poorly characterized.
